Terms and Conditions

Insurance and Ehthics

  • Each therapist who works at or for Gayle Campbell Torto, LMT is licensed and certified. Therapists must abide by the code of ethics set forth by the American Massage Therapy Association (AMTA).

Clients

  • All treatments provided by Gayle Campbell Torto, LMT will last for specified duration of the purchased service; services will not end prematurely with the exception of emergency situations.

  • All clients must complete a client questionnaire prior to treatment.

  • Treatments will only be conducted if therapist deems the treatment safe for the client to receive.

  • Doctor permissions may be required before treatment can be provided.

Young Clients

  • Clients under the age of 18 must be accompanied by an adult.

  • The accompanying adult must give spoken or written permission for the treatment to be performed.

  • The accompanying adult must co-sign the client’s questionnaire.

Privacy

  • Client treatment details will not be discussed with anyone other than the client unless the client is under the age of 18 or has a care worker or guardian.

  • Consultations will only be discussed with the client unless the client is under the age of 18 or has a care worker or guardian.

  • Consultation Forms and treatment notes will be kept on file and secured by Gayle Campbell Torto, LMT.

  • All questionnaires will be secured and will not be shared with anyone accept the client unless the client is under the age of 18 or has a care worker or guardian.

  • By filling out the questionnaire, clients are giving Gayle Campbell Torto, LMT permission to hold records and data about them.

  • Personal information will not be shared with any third parties.

Inappropriate Behavior

  • Therapists have the right to terminate a treatment immediately if the client demonstrates inappropriate or sexual behavior.

  • Payment will be collected for the full treatment price; provided more than 10 minutes has been spent treating the client.

  • Full payment is due upon completion of the session. No exceptions.

cancellation Policy

  • A booking is confirmed when Gayle Campbell Torto, LMT and the client have both agreed on a date, time, duration, and location for a treatment.

  • If a cancellation is made more than 24 hours before a treatment, the client will not be able to change booking information.

  • If a cancellation is made within 24 hours of the treatment, they will be expected to pay a cancellation fee of $40 for each hour cancelled.

  • This cancellation fee may be waived at Gayle Campbell Torto, LMT's discretion.

  • The cancellation fee must be paid within 7 days of the cancellation.

  • Gayle Campbell Torto, LMT has the right to cancel a treatment by giving 24 hours notice and may give less than 24 hours notice under special circumstance.

  • Gayle Campbell Torto, LMT has the right to cancel a treatment without notice if the room or environment is unsuitable for the treatment to be provided, or if the therapist is taken ill.

  • Gayle Campbell Torto, LMT has the right to cancel a treatment without notice if the client is intoxicated or inebriated.

  • Gayle Campbell Torto, LMT commits to rearranging treatments to a new time and date within two weeks of the cancelled treatment where possible​.

Payment & Gift certificates

  • Gayle Campbell Torto, LMT accepts payment in the form of cash, check (made out to Gayle Campbell Torto, LMT), and all major credit cards and debit cards.

  • Gift certificates can be used as payment towards any available, advertised treatments.

  • Gift certificates are nonrefundable and cannot be exchanged for cash, even if the amount for the gift certificate is greater than the service purchased/provided.

Website Content

  • Website content will be authorized and managed at Gayle Campbell Torto, LMT's discretion; including testimonials.

  • Photographs will only be displayed with the consent of the people in the photograph.

  • Photograph consent may be obtained by Gayle Campbell Torto, LMT verbally, or in writing if the client completes a photo release form.

  • Photo release forms can be requested by phone or email.

Complaints Procedure

  • If a client wishes to make a complaint about Gayle Campbell Torto, LMT, the client may contact using the contact section on gaylecampbelltortolmt.com/contact.

  • Complaints must include the date and location of the incident, the full name of the complainant, details of the complaint and a desired outcome following the complaint.

  • All complaints will be taken very seriously and a response will be provided within 28 days.

  • There is no guarantee the requested resolution will be fulfilled and it is up to Gayle Campbell Torto, LMT's discretion to resolve the issue.

Rules of Ethics

The Rules of Ethics are mandatory and direct specific standards of minimally-acceptable professional conduct for all members of the association. The Rules of Ethics are enforceable for all association members, and any members who violate this Code shall be subject to disciplinary action.
